2008 CAT 3 Scuba known sequence

ISF waiver and release
This is a standard waiver and release form to sign and bring with you to the events. If you're Scuba certified, you've seen a form similar to this prior to every class you've taken.
ISF medical information form
This is a standard Scuba medical release. Remember, you must write a 'y' or 'n' for each answer, don't just run a line down the columns. If you answer 'yes' to any of the questions, please call us at 206-434-1313 prior to the event. All medical discussions will be with a Scuba instructor and will be kept in complete confidence.
